What Is Revision Surgery?

Revision surgery is an opportunity to revisit a past enhancement that currently dissatisfies you. In the case of breast implants, some women no longer have the bodily proportions to support the bust size they once desired. It is common for the body to react to the introduction of a breast implant by surrounding it with protective scar tissue. This tissue creates a pocket around the implant, called a capsule. If the capsule surrounding your breast implant becomes hard or uncomfortable, Dr Hamlet can remove part or all of the tissue in a procedure called a capsulectomy.[1] After correcting the shape or feel of a capsule, Dr Hamlet can then proceed with the implantation of a new implant, if that is what you so desire.

En Bloc Procedure

When a doctor removes both the capsule and its encased implant simultaneously, this is called an en bloc procedure. En bloc is a French term that means all together, so it is efficient and safe for a surgeon to remove a breast implant along with its surrounding scar tissue. Dr Hamlet can then move forward with your breast augmentation procedure if you wish to replace the capsule and outdated implant with new material.

In very rare cases, women with breast implants have associated their procedure with the occurrence of anaplastic large cell lymphoma (ALCL). According to an exhaustive study of breast implant recipients, researchers were only able to tentatively correlate 34 cases of ALCL out of approximately 10 million individual cases.[2] Breast implant associated anaplastic large cell lymphoma (BIA-ALCL) is a highly treatable condition, especially when it is diagnosed in its early stages and addressed by a skilled medical professional.[3]

Preparation and Procedure

We tailor the preparation protocols for a breast implant revision to your anatomy and cosmetic needs. As with your first surgery, we administer anaesthesia to perform the revision. When possible, we will also trace the same incision locations from your past procedure to achieve your updated surgical goals.

Breast Implant Revision

Dr Hamlet will remove your existing implants and replace them with new breast implants. To properly place the implants of your choosing, Dr Hamlet might need to contour the breast pocket accordingly. If you have excess scar tissue from your previous breast surgery, the doctor may perform either a partial or a total capsulectomy. 

To provide additional support to the newly placed implants, Dr Hamlet might need to reposition or reinforce your natural tissue with an acellular dermal matrix. This is a mesh-like weave that adds support and structure to the breast pocket, specifically along the lower pole.[4] AlloDerm and Strattice are two of the more popular brands of acellular dermal matrices. 

Once Dr Hamlet has skillfully acclimated the replacement implants in their surroundings, he closes all incisions with sutures. Dr Hamlet covers the breasts with surgical dressing to begin the recuperation process.

Recovery Time

There will be some minor bruising and swelling after surgery. Dr Hamlet will prescribe medication to help you manage any discomfort. After a day or two, you will be welcomed back to Dr. Grant Hamlet to have your bandages removed. We will advise you to wear a supportive bra for at least 2-3 weeks as you heal.

In general, you can resume normal activities 7-10 days after breast revision surgery, but every case varies. Each patient has unique circumstances, so there is a range of recovery times associated with the procedure. You can discuss your individual recovery journey during your one-on-one consultation.
